I will use the analogy of a tree. Plastic surgery is a sub-specialty in the feild of surgery, where surgery is the trunk of the tree and plastic surgery is a branch of the tree. Other branches on this tree are orthopedic surgery, vascular surgery, gynocological surgery, and ect. The leaves on each branch represent sub-specialties within each specialty. For example, some orthopedic surgeons will do hip replacements while others will orthroscopic knee repairs. Some vascular surgeons will focus on large blood vessels while others will work on small blood vessels. Like other specialties, plastic surgery has many leaves on its branch, one of these leaves is cosmetic surgery. To answer your question, there is no better and there is no worse. Cosmetic surgery is a general term used to describe a subset of work that a liscenced plastic surgeon can perform if their training permits.

Cosmetic surgeons generally are specialist of a various things other than plastics such as ear, nose, and throat ENT or oral surgeons (specialized dentists).
