Which of the following is the most likely diagnosis?

A 31-year-old homosexual man presents complaining of pain with defecation. He denies any symptoms of diarrhea, abdominal pain, or fevers. Six months earlier, he developed traveler's diarrhea while vacationing in Mexico. On physical examination, he is afebrile and has an unremarkable abdominal examination. On examination of the perianal area, there is a group of five clustered ulcers adjacent to the anal orifice and extending into the anal canal. A sigmoidoscopy reveals normal rectosigmoid mucosa. Which of the following is the most likely diagnosis?

Ulcerative colitis

could be hemmoroids that have bursted. could be bacterial/viral or a parasite. Could even be an STD. From the information given, I would think that the diarrhea situation had nothing to do with this. It is most probably hemmoroids but please don't rely on anyone's diagnosis here.. We aren't Dr. House or anything (from the TV show "House")
